Purpose

The AstraZeneca Foundation CHANGE Program will provide funding in California, Delaware, Maryland, Massachusetts, and Texas to improve access to quality healthcare for people experiencing health disparities. Organizations must provide services tailored to the needs of their communities to help improve healthcare access while considering social drivers of health that impact community members.

Projects may include, but are not limited to:

  • Providing access to care services and coordination of care
  • Mitigating barriers that impact access to care services
Eligibility

Eligible applicants are nonprofit organizations working to improve healthcare access in California, Delaware, Maryland, Massachusetts, and Texas with total revenue of $12 million or less in the most recent tax year.

Priority will be given to organizations working in rural and/or low-income areas that:

  • Promote screenings, early detection, treatment, and/or continuity of care for cardiovascular, renal, respiratory, or immunologic diseases and/or cancers
  • Establish new partnerships and broader coalitions of community-based organizations or expanding existing partnerships with the intent of breaking down institutional silos and facilitating access to quality healthcare through inter-institutional collaboration

Amount of funding

Award amounts are expected to be between $200,000 - $220,000.

The online application will be available until May 5, 2026. Finalists will be selected and invited to submit a full application that is due July 8, 2026.
