Purpose

The Delta Rural Integrated Health Network Program is offering funding to improve healthcare delivery in the Delta region by supporting the development of integrated health networks among rural hospitals, primary care clinics, behavioral health providers, and other essential healthcare organizations. Networks can include groups of healthcare organizations such as rural hospitals or clinics. There is a particular interest in networks and proposed networks that include both hospitals and clinics.

Program goals:

  • Promote cooperation among healthcare organizations to achieve efficiencies and improve access to quality healthcare services and outcomes in rural areas
  • Strengthen the rural healthcare system by fostering partnerships among healthcare organizations
  • Enhance local capacity to improve rural community health interventions and coordinate care effectively
  • Support the financial stability of member healthcare organizations so they can continue to provide care in their communities

There are 2 program tracks:

  1. Create a new network
  2. Strengthen or expand an existing network

Eligibility

Types of eligible organizations:

  • State, county, city, township, and special district governments
  • Public, state-controlled, and private institutions of higher education
  • Native American tribal governments (federally recognized)
  • Native American tribal organizations (other than federally recognized tribal governments)
  • Nonprofit organizations with and without 501(c)(3) status with the IRS
  • For profit organizations and small businesses

Program activities must target rural healthcare organizations located in the Delta region.

255 counties and parishes across 8 states: Alabama, Arkansas, Illinois, Kentucky, Louisiana, Mississippi, Missouri, and Tennessee
