Purpose

The Telehealth Nutrition Services Network Grant Program offers funding to use telehealth networks to integrate nutrition services into primary and specialty care in rural and Medically Underserved Areas.

Proposed projects must provide nutrition interventions targeting at least 2 chronic diseases, such as:

  • Autoimmune disorders
  • Asthma
  • Allergies
  • Cancer
  • Cardiovascular disease
  • Diabetes
  • Mental health
  • Neurodevelopmental disorders
  • Obesity

Funding may be used for:

  • Developing and delivering telehealth nutrition services that enhance access to community-based healthcare services in rural areas, frontier communities, or Medically Underserved Areas (MUAs), or for Medically Underserved Populations (MUPs)
  • Developing and acquiring, through lease or purchase, equipment that furthers the objectives of the program
  • Developing and providing distance education that enhances access to care in rural areas, frontier communities, or MUAs, or for MUPs
  • Mentoring, precepting, or supervising healthcare providers and students seeking to become healthcare providers in a manner that enhances access to care in the target areas and communities or for the target populations
  • Developing and acquiring instructional programming
  • Communicating medical data and maintaining equipment
  • Compensating specialists, including travel expenses
  • Paying healthcare providers who provide telehealth services
  • Developing projects to use telehealth technology to facilitate collaboration between healthcare providers
  • Collecting and analyzing usage statistics and data to document the cost-effectiveness of the telehealth services
  • Carrying out other activities to achieve the objectives of this program, as approved by HRSA
Eligibility

Eligible applicants include:

  • State, territory, county, city, township, and special district governments
  • Independent school districts
  • Public, state-controlled, and private institutions of higher education
  • Native American Tribal governments
  • Public housing authorities/Indian housing authorities
  • Native American Tribal organizations
  • Nonprofits with or without 501(c)(3) IRS status
  • For-profit organizations
  • Small businesses
  • Faith-based organizations

Activities must support HRSA-designated rural counties and rural census tracts.
