This opportunity offers funding to strengthen dementia-capable community health worker (CHW) programs within the National Aging Network and increase the number of people living with dementia (PLWD) who receive dementia-capable early detection, care navigation, and services.
Program objectives:
- Early detection and care navigation - More PLWD, year over year, screened by CHWs will complete a referral for cognitive assessment and/or appropriate supportive services
- Service engagement and barrier reduction - PLWD receiving CHW support will demonstrate increased engagement with dementia-related services
- Participant well-being - PLWD and their family caregivers will demonstrate measurable improvement in quality of life

Eligible applicants:
- Area Agencies on Aging (AAAs)
- State Units on Aging operating as a single planning and service area
- Tribal, Alaska native, and Native Hawaiian organizations receiving federal funds to carry out Title VI programs
Award ceiling: $250,000 per year
Award floor: $200,000 per year
Project period: 3 years
Estimated number of awards: 7
Estimated total program funding:
$5,250,000
A 25% match of the total project cost is required. Match may come from cash or in-kind contributions.
