Purpose

This opportunity offers funding to strengthen dementia-capable community health worker (CHW) programs within the National Aging Network and increase the number of people living with dementia (PLWD) who receive dementia-capable early detection, care navigation, and services.

Program objectives:

  • Early detection and care navigation - More PLWD, year over year, screened by CHWs will complete a referral for cognitive assessment and/or appropriate supportive services
  • Service engagement and barrier reduction - PLWD receiving CHW support will demonstrate increased engagement with dementia-related services
  • Participant well-being - PLWD and their family caregivers will demonstrate measurable improvement in quality of life

Eligibility

Eligible applicants:

  • Area Agencies on Aging (AAAs)
  • State Units on Aging operating as a single planning and service area
  • Tribal, Alaska native, and Native Hawaiian organizations receiving federal funds to carry out Title VI programs

A 25% match of the total project cost is required. Match may come from cash or in-kind contributions.
