Wisconsin Office of Rural Health Community Heroes
The Wisconsin Office of Rural Health (WORH) Community Heroes are honorary awards for leaders in Wisconsin who have made an impact on rural health in the last year.
Awards will be made in three categories:
- Organization
- Program
- Individual
Eligible nominees are organizations, programs, coalitions, and individuals who have made an impact on rural health in Wisconsin within the last year.
Awards are honorary.
Selected nominations will be featured on WORH social media, the WORH National Rural Health Day webpage, and profiled in the twice monthly WORH newsletter, Rural Health Radar. All Community Heroes will receive a certificate of recognition.
