Michigan Upper Peninsula Community Health Worker Recruitment and Retention Opportunity
Funded by the Rural Health Transformation Program, this opportunity offers funding to expand access to care by supporting selected Upper Peninsula Rural Health Clinics (RHCs) in recruiting or identifying, hiring, integrating, and sustaining one community health worker (CHW). CHWs may support patient outreach, health education, care coordination, resource navigation, social needs screening and referrals, follow-up, and community-based service delivery.
Certified Rural Health Clinics located in Michigan's Upper Peninsula are eligible to apply.
Each selected RHC may receive up to $80,000 to support CHW salary and fringe benefits, along with approved training, certification, mileage, and related project costs.
The grant period begins upon execution of the subgrant agreement and ends September 30, 2027.
