Kansas Community Health Worker Accountable Food Is Medicine Grant
Funded by the Rural Health Transformation Program, this opportunity offers grants designed to improve health outcomes among rural Kansans by integrating community health workers (CHWs) into clinical care teams to connect patients with health and social services to bridge gaps in care. Projects will also establish or expand Accountable Food Is Medicine (AFIM) programs, which integrate nutrition interventions, such as medically tailored meals and produce prescriptions, into healthcare to prevent, manage, or treat chronic conditions.
Program objectives:
- Increase the use of validated screening tools to assess health-related social needs, chronic disease risk, and nutrition-related health
- Improve clinical indicators related to chronic disease
- Increase the availability of AFIM programming
- Increase the number of individuals receiving appropriate referrals to AFIM programming
- Increase self-reported health among AFIM participants
- Increase the number of healthcare organizations employing CHWs and offering CHW services
- Support healthcare organizations in the appropriate integration of CHWs, ensuring CHWs are consistently performing duties aligned with the approved CHW Scope of Work
- Increase adherence to medically tailored treatment plans
- Increase the number of healthcare organizations sustaining CHWs through establishing billing practices, integrating workflows, and communicating value
- Expand coverage of CHW and AFIM services through public and private payment systems
