Purpose

The Medica Foundation Rural Health Program offers funding for organizations to support health related programs that address social determinants of health and strengthen regional and rural communities in Medica's service area.

Eligibility

To qualify, organizations must operate within Medica's service areas in Minnesota, western Wisconsin, North Dakota, and South Dakota. Nonprofit organizations that are 501(c)(3) legal entities or government agencies are eligible to receive funding.

Minnesota applicants must be located outside of the 7-county Twin Cities metropolitan area. Nebraska applicants must be located outside of the Omaha metro area.

$5,000 - $10,000
