Profiles Minnie Hamilton Health Care Center's (MHHCC) progress as part of the Small Rural Hospital Transition (SRHT) Project, completed in 2017, focused on improving quality of care and transitions of care throughout its facilities. MHHCC is an 18-bed, not-for-profit Critical Access Hospital in Grantsville, West Virginia. MHHCC also operates a Rural Health Clinic in Grantsville and a Federally Qualified Health Center in Glenville.
