Purpose

The National Rural Health Resource Center provides technical assistance and consultation services for selected applicants through a 3-year participation cycle. The program will assist rural health leaders and providers with implementation of best practices and evidenced-based models that improve performance and enhance care coordination to develop a local system of care.

Focus areas:

  1. Community care coordination
  2. Healthcare organization consultations

Community care coordination in conjunction with consultation services that promote performance improvement initiatives help leaders to:

  • Position the organization for future success
  • Participate in, and benefit from, value-based care
  • Prepare for population health
  • Sustain access to local healthcare services

  • Quality
  • Telehealth
  • Revenue Cycle
  • Physician Practice Management
  • Employee Engagement
  • Workforce and Leadership
  • Implementation assistance
Eligibility

Eligible participants include Critical Access Hospitals, small rural hospitals, Rural Health Clinics, and other healthcare organizations located in the Delta Regional Authority service area.

Geographic coverage
Counties in Alabama, Arkansas, Illinois, Kentucky, Louisiana, Mississippi, Missouri, and Tennessee that are served by the Delta Regional Authority.
Amount of funding

This is a technical assistance opportunity that is provided at no cost.
