The National Rural Health Resource Center provides technical assistance and consultation services for selected applicants through a 3-year participation cycle. The program will assist rural health leaders and providers with implementation of best practices and evidenced-based models that improve performance and enhance care coordination to develop a local system of care.

- Community care coordination
- Healthcare organization consultations
Community care coordination in conjunction with consultation services that promote performance improvement initiatives help leaders to:
- Position the organization for future success
- Participate in, and benefit from, value-based care
- Prepare for population health
- Sustain access to local healthcare services
Consultations services target:
- Finance and operations
- Quality
- Telehealth
- Revenue Cycle
- Physician Practice Management
- Employee Engagement
- Workforce and Leadership
- Implementation assistance
Eligible participants include Critical Access Hospitals, small rural hospitals, Rural Health Clinics, and other healthcare organizations located in the Delta Regional Authority service area.
This is a technical assistance opportunity that is provided at no cost.
