This opportunity will award 2-4 cooperative agreements to establish a national Center for Indigenous Innovation and Health (CIIH) to address the documented chronic disease burden and healthcare access gaps experienced by American Indian and Alaska Natives (AI/AN) and Native Hawaiian and Pacific Islander (NHPI) populations. Funded projects should implement evidence-based frameworks and community-based strategies to prevent and improve chronic disease outcomes, including obesity, hypertension, diabetes, and related conditions, within one Indigenous population of focus.
Projects funded through this agreement will:
- Address obesity, diabetes, hypertension, and related chronic conditions among AI/AN and NHPI populations using evidence-based and prevention-focused approaches
- Build community-level data capacity and evaluation infrastructure to support rigorous, science-driven measurement of health outcomes among AI/AN and NHPI population
- Support cross-sector partnerships that address root causes of chronic disease, including food insecurity, limited healthcare access, environmental exposures, and health literacy gaps, in AI/AN and NHPI communities with documented unmet need
- Support research, education, service, partnership development, technical assistance, and dissemination activities that strengthen prevention and improvement of chronic disease outcomes
Expected activities include:
- Identify and implement an evidence-based framework for research, education, service, partnership development, and technical assistance to support prevention and the improvement of chronic disease outcomes
- Establish and maintain a structured stakeholder engagement process to inform project implementation, priorities, and quality improvement
- Partner with academic institutions, Indigenous leaders, and AI/AN and NHPI communities on CIIH activities
- Implement a rigorous process and outcomes evaluation
- Collect and report program data
- Document and disseminate project findings
- Conduct a health outcomes and population needs assessment that documents chronic disease burden, service gaps, and community conditions
- Develop a sustainability plan
Public or private nonprofit entities, including faith-based organizations and AI/AN and NHPI organizations, are eligible to apply. Examples of eligible institutions include:
- State and territory governments
- County governments
- City or township governments
- Special district governments
- Federally recognized Native American tribal governments
- Tribal organizations
- Public and state-controlled institutions of higher education
- Private nonprofit institutions of higher education
- Independent school districts
- Nonprofits with or without 501(c)(3) status
- Public housing authorities
- Indian housing authorities
Award ceiling: $1,000,000 per year
Award floor: $500,000 per year
Project period: up to 2 years
