Purpose

The Rural Health Network Advancement Program is offering grants to support integrated healthcare networks and improve the quality of basic healthcare services to strengthen rural healthcare systems.

Integrated healthcare networks should:

  • Collaborate to achieve efficiencies
  • Expand access to, coordinate, and improve the quality of basic health care services and associated health outcomes
  • Strengthen the rural healthcare system as a whole

There are 2 program tracks with specific goals.

  1. Operational track
    • Demonstrating enhanced billing and revenue across member organizations
    • Shared savings
    • Demonstrated improvements in payer negotiation outcomes
  2. Clinical services track
    • Increased patient volume
    • Increased service utilization
    • Improved quality measures
    • Improved health outcomes
    • New or sustained revenue streams for participating providers
Eligibility

Eligible applicants:

  • State, county, city, township, and special district governments
  • Independent school districts
  • Public, private, and state-controlled institutions of higher education
  • Native American tribal governments (Federally recognized)
  • Native American tribal organizations (other than Federally recognized tribal governments)
  • Nonprofit organizations with and without 501(c)(3) status with the IRS
  • For-profit organizations and small businesses
  • Domestic faith-based and community-based organizations
  • Tribes and tribal organizations
  • Federally Qualified Health Centers (FQHCs)
  • Community health centers
  • Rural Health Clinics (RHCs)
  • Hospitals
  • Rural Emergency Hospitals (REH)

Award ceiling: $500,000 per year
Project period: 4 years
Estimated number of awards: 6
Estimated total program funding: $3,000,000
