Learning community in New York designed for hospitals pursuing geriatric emergency department accreditation. The goal is to improve care provided to older adults in the emergency department, establish resources required to provide high-quality care, focus on providing the highest standard of care for older adults in the community. Hospitals in rural communities are encouraged to participate.

New York State Age-Friendly Health Systems Action Community Inactive
Learning community in New York that includes a virtual summit, monthly webinars, and topical coaching calls designed to incorporate evidence-based practices and transform the care given to older adults. Participants include Federally Qualified Health Centers and community-based social service organizations, among others.

Teaching Health Center Planning and Development Program Inactive
Planning and development grants to establish new accredited or expanded community-based primary care residency programs in family medicine, internal medicine, pediatrics, internal medicine-pediatrics, psychiatry, obstetrics and gynecology, general dentistry, pediatric dentistry, and/or geriatrics. The program is designed to support the expansion of the primary care physician and dental workforce in rural and underserved communities.

Kentucky Elder Wellness Enrichment Initiative Grant Inactive
Grants for nonprofit rural healthcare organizations and community-based organizations in Kentucky to create or enhance services for older adults that help them to age in place.

Geriatrics Academic Career Award Program Inactive
Funding to support the career development of junior faculty in geriatrics as academic geriatricians or academic geriatrics specialists. The overall goals of the program are to develop the necessary skills to lead healthcare transformation in a variety of settings, including rural and/or medically underserved settings, to be age-friendly and to provide training in clinical geriatrics, including the training of interprofessional teams of healthcare professionals to provide healthcare for older adults.

Medicare Savings Programs Grant Inactive
Grants to educate, engage, and enroll eligible older adults with low income in Medicare Savings Programs. The program is available in 21 states and Puerto Rico and has a focus on underserved populations, including minorities and those living in rural communities.

Rural Communities Opioid Response Program - Medication-Assisted Treatment (RCORP-MAT) Access Inactive
Grants to establish new Medication-Assisted Treatment access points and increase capacity for the provision of sustainable MAT services in high-risk rural communities, thus increasing access to evidence-based treatment for individuals with opioid use disorder (OUD) living in rural areas.

Native American Elder Justice Initiative (NAEJI) Inactive
Funding to operate a national center that addresses the need for culturally appropriate information and community education materials on elder abuse, neglect, and exploitation in Native American/Alaska Native communities. Activities include developing culturally specific materials for tribes, gathering and disseminating information on best practices, and gathering and disseminating examples and information on model tribal codes to address elder maltreatment.

Reducing Racial and Ethnic Healthcare Disparities in Chronic Conditions by Dissemination and Implementation of Patient Centered Outcomes Research (PCOR) Evidence (R18) Inactive
Funding to disseminate and implement patient-centered outcomes research (PCOR) findings into innovative, evidence-based interventions to reduce healthcare disparities in chronic health conditions. Priority populations include inner city; rural; low income; minority; women; children; elderly; and those with special healthcare needs, including those who have disabilities, need chronic care, and need end-of-life healthcare.
