Greater Minnesota Tribal Transit Vehicle Grant
Funding for tribal transit programs in Minnesota to support replacement and/or expansion of transit vehicles.

Kansas Community Health Worker Accountable Food Is Medicine Grant
Grants designed to improve health outcomes among rural Kansans by integrating community health workers into clinical care teams to connect patients with health and social services to bridge gaps in care. Projects will also establish or expand Accountable Food Is Medicine programs, which integrate nutrition interventions, such as medically tailored meals and produce prescriptions, into healthcare to prevent, manage, or treat chronic conditions.

Oregon Rural Veterans Healthcare Transportation Grant Program
Grants to cover operations and purchased services for eligible transportation agencies in Oregon. Funding may be used to address transit challenges and barriers between veterans living in rural areas and healthcare services.

Texas Colonia Fund - Construction
Funding to support community development projects such as sewer, water, other infrastructure, and housing rehabilitation in colonias in Texas.

Minnesota Small Cities Development Program
Funding to help Minnesota cities with fewer than 50,000 residents and counties with fewer than 200,000 with housing, public infrastructure, and commercial rehabilitation projects.

Blue Cross Blue Shield of Michigan Foundation Student Award Program
Funding for doctoral and medical students at Michigan universities who research healthcare topics, including demonstration and evaluation projects and pilot programs that are aimed at improving healthcare within the state.

Dental Trade Alliance (DTA) Foundation Grants
Funding for projects and research to increase access to and utilization of oral healthcare.

Opioid Settlement Fund Community Grant Program
Grants for organizations working to abate and alleviate the impacts of the opioid crisis and co-occurring substance use challenges in South Dakota communities. Rural behavioral health workforce, medication-assisted treatment (MAT) training, and telehealth in rural communities are among the approved uses of funds.

Roundhouse Foundation Grants
Funding for rural Oregon supporting projects in arts and culture, education, the environment, and social services.

VA Pilot Program for Graduate Medical Education and Residency (PPGMER)
Reimbursement funding to establish new residency programs by facilities operated by Indian Health Service (IHS), Indian Tribes, tribal organizations, Federally Qualified Health Centers (FQHCs), Department of War (DoW), and other healthcare facilities located in areas designated as underserved by VA, including rural and frontier areas.
