Privacy, Security, and Ethical Considerations

Concerns about data privacy, cybersecurity, and legal and policy considerations can complicate the adoption of AI in rural healthcare settings. AI tools can collect, analyze, and store sensitive health information data, making strong data protection, cybersecurity infrastructure, and staff training essential. Rural healthcare organization leaders should have a clear understanding of where and how patient data are stored (for example, local servers, cloud environments, or third-party systems), and for how long they are retained.

Rural organizations may also face added challenges related to liability and patient safety. For example, while AI can support clinical decision-making, healthcare providers are ultimately responsible for these decisions and for using AI tools safely. AI supports but does not replace the clinician's judgment and decision-making. Also, patients must provide informed consent for the use of AI in their care and understand how their information will be used. They retain the right to access their medical records, including information generated with AI. Legal and policy guidance in this area is continuing to evolve.

AI tools are not "set and forget" technologies. Continuous monitoring for performance, safety, and unintended consequences is critical, especially in rural environments or settings where resources are limited.

Establishing risk mitigation strategies can help organizations adopt AI safely. These may include:

  • Establishing governance structures, such as governance boards, local oversight committees, or designated AI leads
  • Assessing AI vendor transparency, including how tools are developed, trained, and updated, where data are stored, and how data are used
  • Monitoring for bias across patient populations
  • Providing staff training on tool use and any limitations
  • Creating feedback loops to identify issues and support monitoring and continuous improvement
  • Partnering with regional health networks to share best practices

These approaches can help rural healthcare organizations ensure that AI tools are used safely and effectively.
