Module 2: Evidence-Based and Promising Aging in Place Program Models

The majority of older adults want to remain in their homes and communities as they age, otherwise known as aging in place. Doing so requires a variety of supportive services to help people remain safe and actively engaged in their communities, even when their health begins to decline. To learn how to identify and adapt interventions, see Developing a Rural Community Health Program in the Rural Community Health Toolkit.

This toolkit identifies eight models that support aging in place, from both an individual and community standpoint.
