Module 3: Program Clearinghouse

The HRSA Federal Office of Rural Health Policy has funded several programs in rural areas with the goal of aging in place as part of the 330A Outreach Authority program. This program focuses on expanding access to healthcare services in rural areas.

Examples of current 330A Outreach Authority grantees and other organizations that have developed promising programs designed to improve aging in place in a rural community are provided below. Evidence-based and promising service models for improving aging in place are available in Module 2.

  • ARcare
    Project Title: Aging Well Outreach Network
    Synopsis: This program works to improve health outcomes and quality of life for Cross County, Arkansas' adults ages 50 and older by reducing fall risks. The program also aims to better manage chronic conditions by coordinating with the Longevity Center and case management.
  • City of Winnemucca, Nevada
    Project Title: Age-Friendly Community
    Synopsis: This program is working with residents of Winnemucca, Nevada to create an age- and dementia-friendly community, with the goal of obtaining an Age-Friendly Community designation through the AARP and the World Health Organization.
  • Michigan Aging & Adult Services Agency
    Project Title: Community for a Lifetime
    Synopsis: This program is open to communities throughout the state of Michigan and supports them in conducting an aging-friendly community assessment, as well as in developing a plan to move toward becoming more aging-friendly.
  • Mountain Empire Older Citizens, Inc. (MEOC)
    Project Title: Mountain Empire Older Citizens, Inc.
    Synopsis: This program works to prevent unnecessary and/or inappropriate institutionalization of older individuals in the community by developing and maintaining its infrastructure to support its community-based long-term care services.
  • National PACE Association
    Project Title: Rural PACE®
    Synopsis: This is a rural-specific branch of the Program of All-Inclusive Care for the Elderly (PACE), designed to provide all medically necessary and long-term services and supports to older adults in the community through the Medicare and Medicaid programs.
  • Onion River Exchange
    Project Title: Onion River Exchange Time Bank
    Synopsis: This program uses a Time Bank service exchange network to organize community-based participation to share services and as well as providing a place to participate as valued members of the community. This serves the physical and social needs of community members, including older adults aging in place.
