Primary Care First Model Inactive
Voluntary 5-year payment options that reward value and quality by offering an innovative payment structure to support delivery of advanced primary care. The model is designed to prioritize the doctor-patient relationship; enhance care for patients with complex chronic needs and high need, seriously ill patients; reduce administrative burden; and focus financial rewards on improved health outcomes.

Medicare Promoting Interoperability Program Hardship Exception Application Inactive
CMS eligible hospitals and Critical Access Hospitals (CAHs) may be exempted from the Medicare Promoting Interoperability payment adjustment if they can show that compliance with the requirement for being a meaningful EHR user would result in a significant hardship. Eligible hospitals and CAHs may submit a hardship exception application each year for a maximum of 5 years.

Emergency Triage, Treat, and Transport (ET3) Model Inactive
A voluntary, 5-year payment model that will provide greater flexibility to ambulance care teams to address emergency healthcare needs of Medicare Fee-for-Service (FFS) beneficiaries following a 911 call. The model is designed to correct misaligned incentives and has the potential to improve the quality of care and lower costs to Medicare by reducing avoidable transports to the hospital ED and potentially reducing avoidable inpatient admissions.
