Community Mobility Design Challenge Grants Inactive
Grants to support planning activities for communities seeking innovative ways to address the particular mobility challenges experienced by low-income community members for whom a lack of transportation is an obstacle to the pursuit of economic, health, and social well-being.

Tribal Maternal, Infant, and Early Childhood Home Visiting Program: Implementation and Expansion Grants Inactive
Funding to implement and expand evidence-based home visiting services in tribal communities. The goals of the Tribal Home Visiting program are to support healthy, happy, successful American Indian and Alaska Native (AIAN) children and families through a coordinated, high-quality, evidence-based home visiting strategy, and to continue to build the evidence base for home visiting in tribal communities.

Rural Health Network Development Program Inactive
Grants for healthcare networks in rural areas to address gaps in service, enhance systems of care, and expand capacity of the local healthcare system. The purpose of this program is to support rural integrated healthcare networks that have combined the functions of the entities participating in the network in order to achieve efficiencies; expand access to, coordinate, and improve the quality of essential healthcare services; and strengthen the rural healthcare system as a whole.

Indian Health Service Substance Abuse and Suicide Prevention Program: Substance Abuse Prevention, Treatment, and Aftercare Inactive
Funding for tribes, tribal organizations, and urban Indian organizations to reduce the prevalence of substance use and decrease the overall use of addicting and illicit substances among American Indian and Alaska Native (AI/AN) populations. Projects should include care coordination, culturally responsive behavioral healthcare services, and early intervention strategies for AI/AN youth at risk for substance misuse behavior.
