  • Need: Expand healthcare access for the more remote residents of 3 frontier counties in north central Idaho.
  • Intervention: Consortium of healthcare providers and community agencies used a hybrid Community Health Worker model to augment traditional healthcare delivery services in order to offer a diverse set of healthcare offerings to frontier area residents.
  • Results: Increased healthcare access, especially for cancer and chronic disease screening, along with providing education on a diverse array of health topics.

  • Need: To reduce the prevalence of diabetes and cardiovascular disease in rural Colorado.
  • Intervention: Community Health Workers are utilized to create a system of coordinated care in Delta, Montrose, Ouray, and San Miguel counties.
  • Results: As of 2018, 2,709 people have been screened for diabetes and cardiovascular disease, with many at-risk patients lowering cholesterol, blood pressure, and A1C levels after engaging with a Community Health Worker.

Jefferson Healthcare Prescription CSA

  • Need: To increase food security among low-income patients living with cancer and diabetes in rural Jefferson County, Washington.
  • Intervention: A local healthcare system partnered with a small organic farm to offer weekly produce boxes to eligible low-income patients.
  • Results: 10 patients participated in the Jefferson Healthcare prescription CSA program in 2022. In 2023, the program grew to serve 15 patients.

  • Need: To help adults and children in rural South Dakota prevent or manage their diabetes.
  • Intervention: The Facing Diabetes Project offered medical visits for adults and provided prevention and education sessions for the local 4th and 5th graders.
  • Results: Many adults and children in the region felt better equipped to choose healthy foods, exercise regularly, and manage their stress: all factors that can help prevent diabetes or decrease its effects.

  • Need: To educate people in rural Virginia who either have diabetes or are considered at high risk for developing it.
  • Intervention: Teleconferencing technology is used to offer diabetes education programs to people with diabetes or those at high risk for developing it. Health professionals are also indirectly trained in diabetes care and management.
  • Results: Participants reported better prevention practices and/or self-management of diabetes after being thoroughly educated about this condition.

  • Need: To access specialty medical services, residents of Santa Catalina Island have to travel 20 miles by boat to southern California. A telemedicine service was needed to connect island residents to the mainland's specialty services.
  • Intervention: The Catalina Island Medical Center created a telemedicine center to give residents quick access to specialty medical care, including diabetic consultations and pain management.
  • Results: An analysis of the telemedicine program found that patients had measurable improvements, and that 90% of patients were pleased with the program and will continue to use it.

  • Need: To help reduce diabetes, depression, and stroke risk in rural residents.
  • Intervention: A collaborative care model was implemented in the Idaho counties of Clearwater, Idaho, and Lewis.
  • Results: Increased number of patients with controlled blood sugar, controlled blood pressure, and higher depression screening rates.

  • Need: Prevention of type 2 diabetes in adolescents living in rural parts of Louisiana.
  • Intervention: Through screenings, the Adolescent Pre-Diabetes Prevention Program detects the onset of prediabetes. Through nutrition and physical activity education, the program teaches high school students and staff how to adopt healthy lifestyles.
  • Results: The program has seen an increase in enrollment and continues to see decreases in body weight, body mass index, and A1C levels among participants.
