The Coffee Break Project

  • Need: Men in the agriculture industry face high suicide rates due to factors including long hours, geographic isolation, lack of social opportunities, and stigma surrounding mental health care.
  • Intervention: The Coffee Break Project, a program led by Valley-Wide Health Systems, Inc. in southeastern Colorado, encourages mental health check-ins for farmers and ranchers through a public awareness campaign and casual coffee gatherings that utilize COMET, an intervention model developed specifically for rural communities.
  • Results: Between eight and 20 people typically attend each coffee gathering.

HIV Telehealth Collaborative Care (HIV TCC) Program

  • Need: To increase access to specialty care for rural veterans living with HIV.
  • Intervention: The HIV Telehealth Collaborative Care (TCC) study connects these patients with HIV specialists via clinical video telehealth or VA video connect and works to create shared care relationships with primary care teams in rural areas.
  • Results: The HIV TCC program provides HIV specialty care access to rural veterans in a sustainable manner with infrastructure, mentorship, and capacity building.

Military Medics and Corpsmen Program

  • Need: To help veterans transition into civilian healthcare careers.
  • Intervention: MMAC and healthcare employers in urban and rural Virginia provide employment and education opportunities to veterans seeking civilian medical credentials.
  • Results: MMAC has been able to assist over 636 veterans, transitioning service members, and military spouses find healthcare employment.

Age-Friendly and Dementia-Friendly Winnemucca

  • Need: To educate rural community members about Alzheimer's disease and other dementias and to support older adults in need.
  • Intervention: A community group formed in Winnemucca, Nevada, to discuss topics like health, housing, social events, community improvement, education, and transportation for older adults. The group also provides outreach to older adults, caregiver support, and a variety of of educational activities and events.
  • Results: The group's efforts have led to many changes for community welfare and safety as well as opportunities for education and activities.

Camp Mariposa

  • Need: To help children whose family members are struggling with substance misuse.
  • Intervention: A year-round program provides mentoring as well as substance use prevention education.
  • Results: In 2022, Camp Mariposa served a total of 123 youth in its four rural locations in Indiana, Kentucky, Tennessee, and West Virginia. In a study, 93% of participants reported no use of any substance to get high.

Implementation of a Nursing Veterans' Initiative to Transform Education (INVITE)

funded by the Health Resources Services Administration

  • Need: To support rural veterans pursuing a career in nursing.
  • Intervention: The INVITE program improved the curriculum and reworked admission requirements to better support veteran students' experiences in the College of St. Scholastica undergraduate nursing program.
  • Results: The number of veterans pursuing nursing has more than doubled since program implementation, and all students have reported an increased interest in serving rural communities.

Lakewood Engage Food Access Initiatives

  • Need: Lakewood, Minnesota is classified as a food desert, where many rural families face food insecurity and low availability of both nutritious and affordable provisions.
  • Intervention: A collection of programs address food insecurity and the dietary health of members of the community, from children and families to older adults, supplying them with locally-sourced fruits, vegetables, and meat.
  • Results: Lakewood Engage conducts around 40,000 food insecurity screenings each year. Fresh produce, meat, and other goods are distributed to roughly 140 families (around 500 individuals), 200 seniors, and 75 school-aged children annually.

Project Swaddle

  • Need: To reduce poor maternal and infant health outcomes and improve access to prenatal and postpartum care for at-risk pregnant women and recent mothers in Crawfordsville, Indiana.
  • Intervention: A home visitation program that uses community paramedics to deliver wrap-around care to women experiencing high-risk pregnancies and/or social and environmental challenges.
  • Results: Since 2018, more than 200 women have been served by the program.

CAPABLE (Community Aging in Place—Advancing Better Living for Elders)

  • Need: To help older adults age in place.
  • Intervention: For five months, CAPABLE participants receive home visits from a registered nurse, occupational therapist, and home repair services.
  • Results: There are currently over 35 CAPABLE sites across the country, 11 of which are located in rural communities.

Clinic for Special Children

  • Need: To provide healthcare for the genetically vulnerable children and adults of the rural, uninsured Amish and Mennonite communities in southern Pennsylvania.
  • Intervention: A clinic that serves as a comprehensive medical practice for children and adults (primarily from the Amish and Mennonite communities) with rare, inherited, or complex disorders.
  • Results: In 2022, about 1,400 active patients with more than 400 unique genetic mutations were treated at the Clinic for Special Children.
