Grants to implement the Screening, Brief Intervention, and Referral to Treatment (SBIRT) model for children, adolescents, and adults in primary care and community health settings with a focus on screening for underage drinking, opioid use, and other substance use.

340B Drug Pricing Program
Allows certain facilities, such as Critical Access Hospitals, Federally Qualified Health Centers (FQHCs), and FQHC Look-Alikes, to purchase prescription and non-prescription medications at reduced cost.

Delta Rural Integrated Health Network Program
Funding to improve healthcare delivery in the Delta region by supporting the development of integrated health networks among rural hospitals, primary care clinics, behavioral health providers, and other essential healthcare organizations.

Maternal Health Emergency Management Training (MHEMT)
Funding to a single entity to provide training development and targeted strategies to address gaps in workforce training and standardize care for healthcare providers in non-delivery and/or low-resource clinical settings, such as first responders, emergency departments, Critical Access Hospitals, and Federally Qualified Health Centers.

Texas Libraries Supporting Community Mental Health
Grant to Texas public libraries to strengthen the mental health and well-being of communities through educational programs, resources, services, and support.

Promoting the Integration of Primary and Behavioral Health Care: States
Funding to improve the health outcomes for people with behavioral health conditions by supporting the adoption and improvement of integrated care models for behavioral and primary physical health at the state level. Tribal, rural, and other medically underserved communities are a program priority. Program eligibility is limited to states and state agencies; however, states must partner at least 1 qualified community program, community mental health center, Rural Health Clinic, or Federally Qualified Health Center.

Small Health Care Provider Quality Improvement Program
Grants to support the planning, development, and implementation of quality improvement activities for rural primary care providers or providers of healthcare services, such as Critical Access Hospitals, Rural Health Clinics, or a network of rural health providers, to increase access to high quality healthcare services and improve health.

Oregon Rural Health Excellence Award
Awards that recognize rural hospitals, clinics, local public health departments, EMS agencies, and community-based organizations that demonstrate exceptional dedication to health outcomes in their communities in Oregon.

Gates Family Foundation Capital Grants
Capital support for building purchase, construction, expansion, renovation, and/or land acquisition for Colorado organizations working in the areas of arts and culture; education; well-being of children, youth, and families; parks and recreation; and community development, among other things.

Rural Physician Associate Program
A 9-month, community-based educational experience for third-year medical students at the University of Minnesota that allows them to live and train in rural communities across Minnesota and western Wisconsin.
