Building Bridges to Better Health: A Primary Health Care Challenge
Challenge competition seeking to advance the development and testing of low-cost, scalable technical assistance solutions that can improve outcomes for patients at Federally Qualified Health Centers. Proposed solutions should focus on addressing key drivers of poor health and social determinants of health, including barriers to accessing medical, social, housing, educational, or other related services.
