Need: To help inform western Wisconsin residents about their health insurance options, community resources and programs, and opportunities to improve their overall health and wellness.
Intervention: The Scenic Bluffs Community Health Centers' Help Team provides free, comprehensive services to help community members overcome barriers to accessing healthcare, including challenges related to health insurance coverage, cost of care, transportation, and other social factors that affect well-being.
Results: In 2024, nearly 1,500 people received support and guidance from the Help Team regarding programs, resources, and health insurance enrollment. In total, 411 individuals were enrolled in health insurance.

Need: To improve sustainability and financial viability for rural healthcare providers throughout Indiana.
Intervention: A network of rural healthcare providers for Critical Access and other hospitals in Indiana that are dedicated to improving their ability to deliver efficient and high-quality healthcare for their rural residents.
Results: The network has been leveraged to increase access to resources, coordinate services, and improve and expand healthcare access.

Need: Organized focus on COPD patients' medical needs to decrease hospital readmissions in a rural Ohio healthcare system.
Intervention: Creation of an integrated system model with nurse navigators central to evidence-based chronic disease care management approaches to COPD care.
Results: Since its creation in 2014, the model continues to mature its comprehensive approach to provide optimized acute and chronic care for the area's COPD patients.

Need: To improve maternal and birth outcomes in rural and underserved areas by increasing the number of family medicine physicians in these areas who have high-quality, evidence-based obstetrical care skills.
Intervention: The STRETCH-OB program trains a select number of family medicine residents at the University of Illinois College of Medicine Rockford each year to provide high-quality maternity care, including surgical obstetrical care.
Results: Four STRETCH-OB residents have graduated as of June 2025 and all have cesarean section privileges in their practice.

Need: To connect people in need in Allegany County, Maryland, to health and human services and to items like hygiene products, food, and clothing.
Intervention: The Possibility Shop partners with health organizations, insurance navigators, food banks, and other agencies.
Results: In 2024, 15,246 service encounters occurred and 940 intakes to services were performed.

Need: To help ensure the viability of and improve quality at 9 rural, independent hospitals serving 11 Utah counties.
Intervention: A network organization was created to allow member hospitals to communicate, network, and undertake projects together.
Results: Members take advantage of cost savings, education, and networking opportunities through group projects and programs.

Need: To help hospitals in rural Idaho and Wyoming see how their prices compare to others in the region.
Intervention: The Hospital Cooperative (THC) completes an annual charge comparative, which lists the highest, lowest, average, and median price for a specific charge.
Results: THC has offered these reports since 2010.

Need: Critical Access Hospitals in Idaho were not able to afford a fixed MRI system.
Intervention: The hospitals partnered to purchase a mobile MRI unit to travel among facilities.
Results: The MRI unit went into service in 2012, providing hundreds of scans per month and traveling among six member hospitals in rural Idaho.

Need: To increase the future healthcare workforce in rural Minnesota.
Intervention: Montevideo Medical Academy helps students explore healthcare career options and gain college credit.
Results: So far, 25 students in the program have attained 170 college credits, 55 certifications, and 304 hours of clinical experience.

Need: In Colorado, 31% of children have experienced dental decay by the time they reach kindergarten. With several frontier and rural counties in Colorado considered Dental Care Health Professional Shortage Areas (HPSAs), options for oral healthcare are limited.
Intervention: Cavity Free at Three (CF3) works to improve access to preventive oral health for pregnant women and young children.
Results: CF3 has trained over 6,000 medical and dental professionals in performing preventive dental health. The percentage of children who received oral healthcare from a medical or dental provider before the age of 2 has progressively increased, and Colorado saw reduced cavities starting in 2007.
