A statewide coalition focused on reducing obesity in Indiana. Focuses on physical activity and nutrition, and other risk factors associated with obesity including communities' food, social, and built environments. A Connections IN Health initiative based in the Indiana Clinical and Translational Sciences Institute (CTSI).
Indiana Area Health Education Centers Network
Works to provide communities throughout Indiana with healthcare professionals through recruitment, education, and retention. Located within the Indiana University of Medicine Department of Family Medicine. Oversees 8 regional Area Health Education Centers (AHECs) in the state. Publishes health workforce studies and reports.
Indiana Center for Rural Engagement
Fosters partnerships between Indiana University Bloomington and rural communities in Indiana through research, cultural assets, and expertise for the purpose of addressing rural challenges.
Indiana Community Action Association
Works to address poverty in Indiana and support low-income individuals who are seeking self-sufficiency. Includes
the Indiana Rural Community Assistance Program (RCAP),
which helps support rural community infrastructure.
Indiana Department of Health
Provides public health services and programs for citizens
of Indiana. Consists of numerous divisions with services
including healthcare quality, minority health, hospital and
acute care licensing and regulations, Rural Health Clinic
(RHC) certification, health and human services, and public

Represents Indiana hospitals and health systems. Includes a
rural health section that lists all Critical Access
Hospitals (CAHs) and county hospitals. Tracks Indiana rural
Indiana Office of Community and Rural Affairs
Oversees numerous programs impacting and serving rural
Indiana communities, including Rural Broadband
Internet, Annual Rural Summit, Community Liaison
Districts, Community Development Block Grants, and the
Stellar Communities Program.
Indiana Office of Primary Care
Housed within the Indiana State Department of Health and
promotes access to primary healthcare services. Administers
the State Community Health Center Operating Grant. Oversees
the State Office of Rural Health and the Health
Professional Shortage Areas (HPSA) and Medically
Underserved Areas/Populations (MUA/P) designations.
Indiana Primary Health Care Association
Works to ensure that all those living in Indiana have
quality healthcare. Provides information on Indiana's
community health centers including a listing of Indiana's
Federally Qualified Health Centers (FQHCs), events,
resources, and advocacy for members.
Indiana Public Health Association
Works to improve and promote the overall health of the
residents of Indiana by providing opportunities for
collaboration, education, professional development, and

Works to improve the health of Indiana residents by
providing education, advocacy, collaboration, and resource
development. Provides information on Critical Access
Hospitals (CAHs), Rural Health Clinics (RHCs), and
emergency preparedness. Serves as the gateway to the
Indiana Statewide Rural Health Network (InSRHN), Indiana
Telehealth Network (ITN), and the Indiana Rural Workforce
Innovation Network (IRWIN).
Indiana State Office of Rural Health
Promotes the rural healthcare workforce and delivery systems in the state of Indiana and helps to coordinate efforts and resources. Supports the Critical Access Hospital (CAH) Medicare Rural Hospital Flexibility (Flex) Program and the Small Rural Hospital Improvement Program (SHIP). Works within the Indiana State Department of Health's Office of Primary Care.

North Central Regional Center for Rural Development
One of four regional rural development centers in the U.S.
that works with extension professionals, researchers, and
partners to enhance rural development outcomes. Serves a 12-state area in the North Central region which includes North
Dakota, South Dakota, Nebraska, Kansas, Minnesota, Iowa,
Missouri, Wisconsin, Illinois, Indiana, Michigan, and Ohio.
Provides consulting and technical support services to help providers in Indiana improve healthcare delivery, measurement, and quality.
Richard G. Lugar Center for Rural Health
Works to prepare and train primary care physicians for rural practice. Promotes rotations in Rural Health Clinics, community health centers, and Critical Access Hospitals. Oversees the Summer Hoosier Rural Preceptorship which immerses medical students in rural underserved communities in Indiana and Illinois.
Upper Midwest Telehealth Resource Center
One of the telehealth resource centers funded by the
federal Office for the Advancement of Telehealth (OAT).
Assists healthcare organizations, healthcare networks, and
healthcare providers in the implementation of
cost-effective telehealth programs to serve rural and
medically underserved areas and populations. Includes
Indiana, Illinois, Michigan, and Ohio.
