- Program Overview: Integrating Professionals for Appalachian Children (IPAC) was a network organization and a Community HUB that managed and oversaw the implementation of the Pathways Program in participating network agencies. These agencies employed Pathways providers that delivered enhanced care coordination services in the community. The IPAC network implemented a Pregnancy Pathway to improve birth outcomes among pregnant women and worked on increasing access to both lactation services and tobacco cessation interventions for Pathway participants. IPAC also piloted an Emergency Department (ED) Utilization Pathway to decrease non-urgent ED usage among children and a Trauma/Foster Care Pathway to increase access to health services for children with histories of trauma and/or foster care placements.
