- Program Overview: The Butte Community Diabetes Network (BCDN) integrated resources for community members with diabetes through case management services. BCDN established a hotline that served as a network referral system for healthcare professionals and consumers. The hotline, managed by the Diabetes Care Coordinator or the Diabetes Educator, provided callers with information about the network's diabetes classes and support groups, made appointments for patients, helped patients obtain needed prescriptions, referred patients to community partners for services and resources, conducted health screenings, and communicated with physicians about their patients. The Diabetes Coordinator also provided education to patients in the hospital as well as one-on-one sessions and/or classes for those with diabetes or at risk. Collaboration and oversight were key to the program’s success.
