Kansas Regional Partnership Grant Program
Grants for eligible providers to participate in collaborative arrangements that promote preventive health and address root causes of diseases, enhance rural providers' efficiency and sustainability, attract and retain a high-skilled healthcare workforce, spark growth of value-based care models, and foster the use of innovative technologies. Funds must be used for the development, coordination, implementation, and evaluation of approved regional partnership activities consistent with program requirements.

Kansas Transformative Capital Investment Grant Program
Grants for eligible providers in rural Kansas to make capital investments to transform the manner in which they serve their communities.

Kansas Community Health Worker Accountable Food Is Medicine Grant
Grants designed to improve health outcomes among rural Kansans by integrating community health workers into clinical care teams to connect patients with health and social services to bridge gaps in care. Projects will also establish or expand Accountable Food Is Medicine programs, which integrate nutrition interventions, such as medically tailored meals and produce prescriptions, into healthcare to prevent, manage, or treat chronic conditions.

Kansas Non-Emergency Transportation (NEMT) Program Grants
Grants to support innovative, sustainable local transportation solutions to help rural Kansans reach medical appointments without duplicating existing services eligible for reimbursement. By improving access to timely care, the program is designed to prevent manageable health issues from becoming emergencies and reduce pressure on rural emergency departments and health systems.

Kansas Rural Health Transformation Program Technical Assistance Hub
A clearinghouse for technical assistance (TA) for organizations applying for and receiving funding through the Kansas Rural Health Transformation Program (RHTP). Technical assistance is designed to help applicants strengthen their understanding of program requirements, work through questions, and develop stronger application materials and offers TA through the grant period.
