• Program Overview: The United Ambulance Service Home Visit Program delivers services, including in-home inspections, safety checks, medication reconciliation, well-being checks, vaccinations, health screenings, chronic disease management education, and wound care, to participants in its community paramedicine program.
  
  Community paramedics also connect patients with a primary care provider if they do not already have one. The patient population includes individuals who have limited resources and limited access to home healthcare, often because they lack comprehensive insurance coverage. To expand support, the United Ambulance Service partners with other organizations, including mental health and substance use services and Meals on Wheels, to connect patients to the resources they need. United Ambulance Service requires that all their community paramedics are CPC or CPT trained.
