Supporting Vulnerable and At-Risk Youth Transitioning Out of Foster Care
Funding for pilot demonstration projects to develop, implement, and build replicable treatment models for residential-based innovative care, treatment, and services for adolescents and youth transitioning out of foster care who have experienced a history of foster care involvement, child poverty, child abuse or neglect, human trafficking, juvenile justice involvement, substance use or misuse, or gang involvement. The program has an interest in providing services for youth in rural areas.

Treatment for Individuals with Serious Mental Illness, Serious Emotional Disturbance or Co-Occurring Disorders Experiencing Homelessness
Grants to eligible entities to provide comprehensive, coordinated and evidenced-based services for individuals, youth, and families with a serious mental illness, serious emotional disturbance, or co-occurring disorder, who are experiencing homelessness or at imminent risk of homelessness.

Community Economic Development Projects
Grants to Community Development Corporations (CDCs) for projects designed to address the economic needs of low-income individuals and families through the creation of employment and business opportunities, including expansion or construction of clinics and health centers. Priority points will be given to projects that create jobs in rural communities and counties of persistent poverty.

Treatment, Recovery, and Workforce Support Grants
Grants to implement evidence-based programs to support individuals in substance use disorder treatment and recovery to live independently and participate in the workforce. Projects should focus on equity for rural, tribal, and other underserved populations.

Stand Down Grants
Provides funding for events that offer homeless veteran populations a variety of social services designed to help them to reintegrate into their communities, such as housing, healthcare, and employment opportunities.

3RNET Recruitment and Retention Assistance
Helps health professionals find jobs in rural and underserved areas and assists employers in recruiting and retaining health professionals in rural and underserved areas.

Business and Industry Loan Guarantees
Loan guarantees to improve, develop, or finance business, industry, and employment and improve the economic and environmental climate in rural communities.

California Advancing Wellness Grant Program
Grants for nonprofit organizations in California working in the areas of healthcare equity and access, community well-being, economic security, and community leadership for change. Grantmaking focuses on the health needs of traditionally underserved populations, including low-income individuals, people of color, youth, and residents of rural areas.

Foundation For the Mid South Grants
Grants for organizations in Arkansas, Louisiana, and Mississippi working in the areas of health disparities, physical health, mental health, education, financial security, and the growth and prosperity of communities.
