Effective Examples
Franklin Cardiovascular Health Program (FCHP)

- Need: To develop sustainable, community-wide prevention methods for cardiovascular diseases in order to change behaviors and healthcare outcomes in rural Maine.
- Intervention: Local community groups and Franklin Memorial Hospital staff studied mortality and hospitalization rates for 40 years in this rural, low-income area of Farmington to seek intervention methods that could address cardiovascular diseases.
- Results: A decline in cardiovascular-related mortality rates and improved prevention methods for hypertension, high cholesterol, and smoking.
Community-Based Pulmonary Rehabilitation Program

- Need: More evidenced-based chronic lower respiratory disease management options for rural Appalachia patients, where lung disease rates are among the highest in the country.
- Intervention: Implementation of outpatient pulmonary rehabilitation programs in 2 Federally Qualified Health Centers and a Critical Access Hospital in West Virginia.
- Results: Improved health outcomes for patients with chronic lower respiratory disease, including those with chronic obstructive pulmonary disease.
Promising Examples
Arkansas Rural Health Partnership Hospital-based Transitional Care Program

- Need: Solutions for Medicare beneficiaries' post-acute care recovery gaps in Arkansas's southeast Delta Region.
- Intervention: Supported by federal funding and their membership organization, seven hospitals implemented an evidence-supported Critical Access Hospital transitional care model.
- Results: Participating hospitals found a significant increase in swing bed services revenue, an all-cause low readmission rate, high percentage of patients discharged to home or to an assisted living environment, and positive patient satisfaction surveys.
School-Based Health Center Dental Outreach

- Need: To improve the oral health status of children ages 3 to 17 living in rural areas of Louisiana.
- Intervention: School-based nurse practitioners perform oral health assessments, apply fluoride varnishes when indicated, and make dental referrals, with completion rates of the latter tracked by dental case managers.
- Results: Significant numbers of school children are receiving oral health examinations, fluoride varnish applications, and receiving care coordination to improve numbers of completed dental appointments.

CMH Addiction Recovery Program
Updated/reviewed November 2025
- Need: To help individuals and families impacted by substance use disorder access treatment and counseling in rural Missouri.
- Intervention: The CMH Addiction Recovery Program provides medication-assisted treatment, counseling, peer and family support, and other related services within a Rural Health Clinic.
- Results: The program operates 5 days a week and sees 400 patients each month.
Mobile Integrated Healthcare Network (MIHN)

- Need: To bring preventive care and other services to rural Missouri patients with chronic illnesses and difficulties accessing primary care.
- Intervention: Community paramedics make home visits and provide basic care, home assessments, and medication reconciliation and facilitate telehealth visits.
- Results: Patients experienced improved access to care, health status, and compliance with medication regimens along with increased patient engagement, satisfaction, and access to community resources.
Scenic Bluffs Community Health Centers Help Team

- Need: To help inform western Wisconsin residents about their health insurance options, community resources and programs, and opportunities to improve their overall health and wellness.
- Intervention: The Scenic Bluffs Community Health Centers' Help Team provides free, comprehensive services to help community members overcome barriers to accessing healthcare, including challenges related to health insurance coverage, cost of care, transportation, and other social factors that affect well-being.
- Results: In 2024, nearly 1,500 people received support and guidance from the Help Team regarding programs, resources, and health insurance enrollment. In total, 411 individuals were enrolled in health insurance.
Indiana Statewide Rural Health Network (InSRHN)
Updated/reviewed September 2025
- Need: To improve sustainability and financial viability for rural healthcare providers throughout Indiana.
- Intervention: A network of rural healthcare providers for Critical Access and other hospitals in Indiana that are dedicated to improving their ability to deliver efficient and high-quality healthcare for their rural residents.
- Results: The network has been leveraged to increase access to resources, coordinate services, and improve and expand healthcare access.
McPherson Hospital High School Internship Program
Added September 2025
- Need: To provide high school students with hands-on health care experience while filling high-need roles in a rural hospital.
- Intervention: An internship program for high school students at McPherson Center for Health in McPherson, Kansas.
- Results: More than 30 students have participated in the program since 2023, seven of whom have become part-time or full-time employees at the hospital as a result.
Montana Interfacility Blood Network (MT IBN)
Updated/reviewed July 2025
- Need: To provide blood transfusions in the ambulance on the way to a hospital.
- Intervention: The Montana Interfacility Blood Network includes 48 healthcare facilities that stock blood and allow ambulances to pick up units of blood while transporting a patient.
- Results: Since its launch, the network has helped at least three patients.
