Awards for individuals and organizations who have made substantial contributions to rural health in Pennsylvania.

Virginia Youth Substance Use Prevention Grants
Grants to support youth substance use prevention instruction in Virginia. Awards will be made statewide.

Michigan Rural Health Clinic Chronic Disease and Collaborative Care Fund
Funding to help Rural Health Clinics build coordinated, sustainable approaches to chronic disease prevention and management based on clinic and community priorities. Projects may address diabetes, hypertension, behavioral health, preventive care, and other chronic conditions and patient needs across the lifespan.

Quimby Family Foundation Grants
Grants for projects focused on physical, emotional, social, and spiritual well-being in Maine. Grants work towards preventing and treating chronic diseases and conditions by addressing environmental connection, isolation and loneliness, sedentary lifestyles, and food systems. Historically, projects in rural communities have been considered a priority.

Tribal Colleges Extension Program Special Emphasis
Funding for special emphasis extension projects at 1994 Land-Grant Institutions. Projects allow extension offices to be responsive to community needs through and address emerging issues such as agriculture productivity, public health and well-being, engaging tribal youth, economic development, and preparing the next generation of tribal agriculture leaders.

Maryland Rural Impact Awards
Honorary awards to recognize individuals and organizations that have provided exemplary service to communities in rural Maryland.

Iowa Farm Bureau Sponsorship Program
Sponsorship support for events and fundraisers in rural Iowa. Giving priorities include health and wellness, agriculture, farm families and their communities, and youth education/leadership.

Kansas Regional Partnership Grant Program
Grants for eligible providers to participate in collaborative arrangements that promote preventive health and address root causes of diseases, enhance rural providers' efficiency and sustainability, attract and retain a high-skilled healthcare workforce, spark growth of value-based care models, and foster the use of innovative technologies. Funds must be used for the development, coordination, implementation, and evaluation of approved regional partnership activities consistent with program requirements.

Kansas Transformative Capital Investment Grant Program
Grants for eligible providers in rural Kansas to make capital investments to transform the manner in which they serve their communities.
