Scaling Evidence-Based Falls Prevention Programs
Funding to help communities reduce falls and/or falls risk among older adults and adults with disabilities through the implementation of evidence-based falls prevention programs. Projects must engage populations age 60 and older, prioritizing those with the greatest social and economic need, including rural areas.

Law Enforcement Mental Health and Wellness Act Program
Funding for state, local, tribal, and territorial law enforcement agencies to implement new or enhance existing programs that offer training and services to support officers' emotional and mental health, including counseling programs, peer mentoring, suicide prevention, stress reduction, and police officer family services. The program intends to increase grant funding accessibility for small and understaffed departments, especially those in rural communities, to implement mental health and wellness programs.

Delta Dental Foundation Community Mini-Grants Program
Funding for community-based oral health programs in Michigan, Ohio, and Indiana, with special consideration given to programs that promote and/or provide oral healthcare access.

Food Distribution Program on Indian Reservations Nutrition Education Grants
Funding to provide nutrition education for participants of the Food Distribution Program on Indian Reservations, resulting in healthier food choices and a better understanding of healthy food preparation methods.

Native Collective Research Effort to Enhance Wellness (N CREW) Program: Addressing Overdose, Substance Use, Mental Health, and Pain
Funding for community-led research to enhance culturally grounded, strengths-based, effective, and sustainable interventions addressing overdose, substance use, mental health, and pain in Native communities.

WHEDA Foundation Housing Grant Program
Supports the development or improvement of housing facilities in Wisconsin for people experiencing a housing crisis.

Alzheimer's Disease Programs Initiative (ADPI) - Dementia Capable Community Health Worker Programs
Funding to strengthen dementia-capable community health worker programs within the National Aging Network and increase the number of people living with dementia who receive dementia-capable early detection, care navigation, and services.

Alzheimer's Disease Programs Initiative (ADPI) - State Programs for Dementia Specific Respite
Funding to support respite for family caregivers of older adults and people with disabilities so they can live well at home and in the community. Those residing in rural areas are among the target populations.

Small Ambulatory Program for American Indians and Alaska Natives
Funding to construct, expand, and/or modernize tribal owned ambulatory healthcare facilities. Facilities must serve American Indian and Alaska Native people and be operated pursuant to a healthcare services contract or compact entered into under the Indian Self-Determination and Education Assistance Act.

Tribal Self-Governance Negotiation Cooperative Agreement Program
Cooperative agreements that provide negotiation resources to tribes interested in participating in the Tribal Self-Governance Program (TSGP), which gives tribes the authority to manage and tailor healthcare programs in a manner that best fits the needs of their communities.
