Types of Program Models
This toolkit identifies six models of community health worker (CHW) programs. These program models are not mutually exclusive; some programs may incorporate elements from multiple program categories. When designing a rural CHW program, it may be appropriate to adapt components from more than one program model to address community needs.

Member of Care Delivery Team Model

Care Coordinator/Manager Model
