Missouri Highlands Delta Care Coordination

  • Program Overview: The Delta Care Coordination program utilizes community health workers (CHWs) to provide care to people with chronic conditions and no health insurance. CHWs conduct assessments, provide screenings, arrange transportation to appointments, provide education, and support patients with any other care navigation needed. The CHWs are located within the Missouri Highlands Health Care clinics and work closely with the healthcare providers as part of a comprehensive care team.

    The program is designed to address emerging challenges by using local needs assessments and involving the community in programmatic choices. For example, when one community needs assessment identified a lack of knowledge surrounding existing resources, CHWs supplied several weather-resistant bulletin boards that allowed community members to view available resources all year. When a need for diabetes education was identified, the CHWs altered their approach and began offering prevention classes.
