- Program Overview: The Kentucky Homeplace program trains community health workers (CHWs) to connect residents of rural eastern Kentucky with appropriate health, environmental, and social services. CHWs provide a range of services, such as diabetic supplies, eye exams and eyeglasses, home heating assistance, and enrollment services. They also provide education and health coaching. The program emphasizes preventive care, health education, and disease self-management.
