  • Program Overview: The Family Wellness Program at Westchester-Ellenville Hospital was designed to reduce childhood obesity, overall rates of chronic disease, and teen pregnancy in the community. One component of the program provides a community health worker (CHW) for families with children ages 5 to 17 who are obese or at risk of becoming obese. The CHWs help families with activities such as community resource navigation, nutrition counseling, and group health education. The CHWs also refer families to primary care, mental health, and dental services as needed. The Family Wellness Program offers additional health promotion services, including a Walk with Ease arthritis management program, an Expanded Food and Nutrition Education Program (EFNEP) in partnership with Cornell Cooperative Extension of Ulster County, and services in partnership with Planned Parenthood.
