Rural Health Care Outcomes Accelerator
A 3-year initiative designed to eliminate rural health disparities by helping hospitals and clinicians provide high-quality evidence-based care. Assistance includes free access to evidence-based programs, consultants, networking, and recognition.

Rural Healthcare Provider Transition Project (RHPTP)
Technical assistance and training for small rural hospitals and certified Rural Health Clinics to build capacity in value-based care (VBC), with emphasis on efficiency, quality, patient experience, and safety. The program's goal is to guide small rural hospitals and Rural Health Clinics that are not currently participating in VBC to prepare for participation in a health system focused on value.

Rural Hospital Stabilization Program
Technical assistance for rural hospitals to help improve financial stability by enhancing or expanding healthcare services that meet community needs.

Substance Use Disorder Treatment and Recovery Loan Repayment Program (STAR LRP) Site Activation
Apply to be an approved Substance Use Disorder (SUD) Treatment and Recovery Loan Repayment Program site to receive assistance with recruitment and retention of qualified SUD clinicians and paraprofessionals.

Rural Health Network Development Planning Program Inactive
Grants to promote the planning and development of integrated rural healthcare networks in order to achieve efficiencies; expand access to, coordinate, and improve the quality of essential healthcare services; and strengthen the rural healthcare system as a whole.

Rural Health Care Services Outreach Program Inactive
Grants to support projects that will expand, enhance, and sustain the delivery of effective healthcare in rural communities.
