Care Coordinator Model

Care coordinators are clinical or non-clinical healthcare workers who provide a range of services in order to help patients overcome barriers to receiving care or treatment. Care coordinators can help to address barriers related to language and culture, communication, transportation, bias (e.g., based on culture, race, or age), and fear. Care coordinators can also help patients transition between healthcare providers and healthcare settings.
