  • Program Overview: Promoting Steps to Wellness is a program established by One Community Health to improve health and prevent chronic disease by providing education on diet, physical exercise, and stress management. This program is conducted by a consortium of partners, including La Clinica del Carino, The Next Door Inc.’s Nuestra Communidad Sana, Providence Hood River Memorial Hospital, and Klickitat County Health Department. This 12-week wellness program targets low-income and medically underserved people speaking English or Spanish and living in a four-county service area. This program also provides diabetes case management. Bilingual and bicultural health promoters were recruited and trained by a registered nurse to conduct diabetes case management. These teams provide case management and education to patients in a culturally appropriate manner.
