Evaluation Objectives

Care coordination programs should develop evaluation objectives that align with the evaluation questions. These objectives should also be SMART:

  • Specific – Detecting changes in the program.
  • Measurable – Collecting data that are complete and accurate.
  • Actionable – Showing performance over time to identify when or where action should be taken.
  • Relevant – Focusing on only the relevant indicators.
  • Timely – Capturing data in the period it is needed, if not real time.
