  • Program Overview: The goal of Community Connections Care Coordination program is to increase access to primary care and social support services for uninsured adults in Grant and Hidalgo counties. Hidalgo Medical Services, a federal qualified health center, and its consortium partners are working together to develop the infrastructure and capacity to integrate primary care, hospital based, mental health, and community support services in the community. Care coordinators work with consortium members to coordinate and address social service needs of participants, which may include housing, medication assistance, cash assistance, publicly funded health insurance, and transportation. Hidalgo Medical Services is using a PCMH model to improve care coordination.
