  • Program Overview: The goal of the Bridges to Health program is to decrease barriers to care by connecting migrant farmworkers to health services. The program uses care coordinators and community health workers (promotores) to provide the following services to migrant farmworkers: transportation, translation services, scheduling of medical appointments, and education about local health services. Promotores also work to improve cultural competency and access to care by suggesting strategies and providing feedback to rural healthcare providers. The program was established by a partnership between the Bi-State Primary Care Association, the University of Vermont Extension, and the Open Door Clinic.
