• Program Overview: Public Health Solutions District Health Department, in partnership with the six critical access hospitals across five rural counties, implemented a patient navigation and case management program to decrease emergency room utilization. The program, Healthy Pathways, focuses on patients with chronic conditions and poor self-management or compliance to increase use of preventive services and a medical home. Case managers work with patients to provide health education and identify the barriers to self-care, with an emphasis on non-medical factors such as access to transportation.
