Module 6: Dissemination of Rural Care Coordination Program Best Practices
Disseminating program results can help rural care coordination programs to build relationships with project
partners and funders, increase visibility, share best practices, and attract funding. Rural care coordination
programs disseminate their projects in a variety of ways. A common starting point is to share findings and best
practices with the program’s board (if applicable), project partners, and other stakeholders. Rural programs
also disseminate by presenting findings to different organizations and sharing materials.
