  • Program Overview: Northeast Missouri Health Council, Inc., in partnership with local providers and health departments in six rural Missouri counties, is implementing a program to increase access to preventive oral health services for pregnant women, infants, children, and special needs residents. The program is promoting a dental home model, which emphasizes an ongoing relationship between the dentist and the patient and the delivery of oral healthcare in a coordinated, family-centered way. A care coordinator supports this effort by managing patient referrals, providing patient navigation support, conducting educational activities, and engaging with stakeholders.
