  • Program Overview: Horizon Health is a nonprofit organization that takes a holistic approach to providing MAT services in their one-stop shop program model. Located in Eastern Kentucky, the “recovery campus” includes a 10,000 square foot facility that provides primary care, case management, and behavioral health, among other services.

    The program coordinates transportation and housing — residential, transitional, and recovery — for patients, and also offers licensed, onsite child care services as well as onsite lab testing. Patients receive behavioral health services in the form of individual and group therapy, and Horizon Health follows an evidence-based treatment plan customized to each patient. The program is accredited through CARF (Commission on Accreditation of Rehabilitation Facilities).

    The majority of patient referrals are by word of mouth, but Horizon Health's social media pages (Facebook, Instagram, and X) and webpage allow for widespread communication and awareness of the program. Program partners, including a local Federally Qualified Health Center, Celebrate Recovery, Cumberland River Behavioral Health, and Volunteers of America also help spread awareness and refer patients to the program. Horizon Health has opened a second facility in Whitley County, which has enabled the program to expand their reach and help additional patients seeking services.
