  • Program Overview: The Southwest Montana Community Health Center (CHC) has integrated substance use disorder services within their existing primary care setting. Using evidence-based models, such as Screening, Brief Intervention, and Referral to Treatment (SBIRT), the CHC links patients with substance use disorders to treatment, referrals, and counseling in a timely manner while in a primary care setting. In addition, the CHC has established a community consortium with key partners and conducted a community-wide anti-stigma campaign.
